To request accommodations for a certification candidate, a school district employee must complete and submit the form on this page. An iCEV representative will contact the district contact with instructions after receiving the form.
If your district requires parental consent to share 504 and IEP information, it is the district’s responsibility to obtain that consent before requesting accommodations from iCEV. Additionally, it is the district’s responsibility to verify ELL status and needs, medical status and needs, 504 Plan status and needs, and/or IEP status and needs and request accordingly. The district should keep certification candidates’ records that correspond with this request for at least one year after the request is made. iCEV reserves the right to audit accommodations requests. ELL Plans, Medical need proof, 504 Plans, and IEP plans may be requested.
